By carefully palpating the side of the neck, one can locate the processus transversus of C3, C4, and C5. To find the processus transversus of the upper vertebra, palpate and identify the processus transversus of the lower vertebrae on the contralateral side. Afterward, perform a slight rotation, make a shift, and apply appropriate flexion or extension.
Instruct the person to sit upright in a relaxed posture. Use your thumb to palpate towards the processus spinosus of C2, and from there, locate the processus spinosus of C3 and the processus transversus of C3. Position your thumb on the processus transversus of C3 and the fingers of the same hand on the contralateral side on the processus transversus of C4.
Approach the patient with the sternum (xiphoid process/incinatum) and place it against the dorso-lateral side of the head. The other hand approaches the processus transversus of C3 from the cranial and dorsal side. By initiating a forward and leftward shift, applying compression on the dorso-lateral side of the head, and simultaneously rotating the upper vertebra to the right, a B typology is induced. The primary forces should arise from the compression on the head and the rotational force. The therapist's fingers mainly guide the movement into the left front position with a rotation to the right.
Palpate with the right hand towards the processus transversus of C3 and hold C4 with the left hand using a pincer grip. Approach the head with the sternum (os xyphoideum/incinatum) on the dorso-lateral side. Initiate, using the right hand, a rotation to the right, and simultaneously perform a shift of the cervical spine in the dorso-lateral direction.
Have the person use their eyes during the mobilization and look in the direction corresponding to the typology being executed for a better utilization of the chain of movement.
