While the person sits upright in a relaxed position, the therapist begins by palpating the first processus spinosus, specifically belonging to C2, using the thumb. Subsequently, the thumb and index finger function as a tweezer to identify the processus transversus of C2, located laterally to the processus spinosus. To complete the rotation of C0C1, the homolateral processus transversus of C2 is gently pushed in the ventral direction, allowing the biconcave condyles to pass each other.
During the mobilization process, the person's head is manually rotated left and right to assess the Range of Movement (ROM). The therapist, positioned on the lateral side of the head with the sternum at the xiphoid level, employs a pincer grip with the thumb on the spinous process of C2 and the index finger stabilizing the arch of C2. Standing close to the patient, the therapist uses the other hand and body to rotate C0 and C1 relative to C2.
To facilitate movement, the patient is instructed to inhale and then exhale slowly, promoting muscle relaxation during the mobilization. Throughout this process, the person is asked to look in the same direction with their eyes, ensuring utilization of a part of the movement chain.
